Abstract: Endocrine disorders are increasingly encountered in pregnancy. To optimize pregnancy outcome, it is essential to understand the physiology underlying these conditions, as well as which investigations and treatments are safe to use. Thyroid disease is the second most common endocrine condition encountered in women of childbearing age after diabetes. Other endocrine disorders, such as pituitary dysfunction and adrenal and parathyroid disease, are less frequently encountered in pregnancy due to lower population prevalence in combination in some cases with associated subfertility. Women whose pregnancies are complicated by endocrine disease are at risk of maternal and foetal complications, but these can be minimized with appropriate multidisciplinary management. PTH stimulates osteoblastic cells to form new bone and to produce osteoblast-osteoclast coupling factors such as RANKL. Whether osteoclasts or their activity are needed for PTH anabolism remains uncertain. We treated ovariectomized huRANKL knock-in mice with a human RANKL inhibitor denosumab (DMAb), alendronate (Aln), or vehicle for 4 weeks, followed by co-treatment with intermittent PTH for 4 weeks. Loss of bone mass and microarchitecture was prevented by Aln and further significantly improved by DMAb. PTH improved bone mass, microstructure, and strength, and was additive to Aln but not to DMAb. The protocols should be tested with known phantoms to determine any differences in sensitivities for detecting small active structures.
